Medical Staff
Richard C. Stephenson, MD

Dr. Stephenson is a graduate of Tufts University and the University of Maryland School of Medicine. Trained in both Internal Medicine and Oncology, Dr. Stephenson has been actively involved in hospice and palliative medicine for over twenty years. He joined the Hospice & Palliative CareCenter in 2000 and is currently the Chief Medical Officer of Medical Services, directing all the activities of the Medical Staff Services Department. Dr. Stephenson is also an Associate Clinical Professor of Medicine at Wake Forest University School of Medicine and developed the Hospice Rotation for 3rd year medical students at the Kate B. Reynolds Hospice Home. Dr. Stephenson is particularly interested in pain management, the development of palliative care services, and end-of-life care education for medical students and house officers.
Karen L. Cross, MD, FAAHPM

Dr. Cross is a graduate of University of Nevada, Reno and the University of Nevada School of Medicine. Dr. Cross, board certified in both Internal Medicine and Hospice & Palliative Medicine, has been actively involved in hospice and palliative medicine for the past twenty years, first in Nevada, and now in Winston-Salem. She joined Hospice & Palliative CareCenter in 2006 as a Medical Director. Dr. Cross has served on the board of directors of the American Academy of Hospice & Palliative Medicine for 8 years and is currently an Associate Clinical Professor of Medicine at Wake Forest University School of Medicine. Dr. Cross is interested in pain and symptom management for hospice and palliative care patients and is currently working to develop a palliative care program with Wake Forest University Health Sciences ALS Clinic.
Freda Clark-Cowan, MSN, FNP-BC

Freda is a graduate of Western Carolina University, UNC-Greensboro, and Old Dominion University. She currently holds two post Master’s certificates in Geriatric and Family Nurse Practitioner with a Master’s degree in Clinical Nursing. Freda has been a member of the Hospice & Palliative CareCenter team since December 2004. She is also wound care certified and provides palliative wound care for hospice and palliative care patients in all settings. She serves patients as a primary care provider along with the physicians at Kate B. Reynolds Hospice Home. She also serves home care patients to meet their palliative care needs. Freda is particularly interested in palliative care services, wound management, and advancing the knowledge of palliative care in the field of nursing.
Cynthia Leonard, MSN, APRN-BC

Cindy is a graduate of Gardner-Webb University and UNC-Greensboro with a Master’s degree in Adult and Geriatric Nurse Practitioner. She has been employed with Hospice & Palliative CareCenter since August 2006. As a nurse practitioner, she serves patients as a primary provider at the Kate B. Reynolds home to meet their medical needs and assist with palliative care efforts for the homecare patients. Cindy is interested in providing optimal pain and symptom management for the patients, advancing the knowledge of palliative care in the field of nursing, and providing research to support palliative therapies.
Kimberly Case, MD

Dr. Case is a graduate of Cornell University and Texas Tech University School of Medicine. She completed a residency in Family Medicine at Wake Forest University Baptist Medical Center and then went on to a fellowship in Hospice & Palliative Care. After completing her fellowship at the Hospice and Palliative CareCenter, she accepted a permanent position in 2008 as a Medical Director. Dr. Case is also an Assistant Professor in the Family Medicine Department at Wake Forest University, where she both sees patients and participates in the teaching of the residents. She is board certified in both Family Medicine and Hospice and Palliative Care. Her special interests include pediatric palliative care and teaching.
Myriah Cox, NP-C

Myriah is a graduate of UNC-Greensboro with a Master’s degree in Adult and Geriatric Nurse Practitioner. She has been employed with Hospice & Palliative CareCenter since September, 2009. As a nurse practitioner, she serves patients as a primary provider at the Kate B. Reynolds Hospice Home to meet their medical needs and assist with palliative care for the homecare patients. Myriah is interested in optimizing quality of life in palliative care and symptom management for the patients, providing palliative care to dementia patients, and advancing the knowledge of palliative care in nursing.
Jennifer Davis, MD

Dr. Davis received her Bachelor of Science degree in Microbiology at Mississippi State University and a Doctor of Medicine degree from the University of Texas Medical Branch. She completed her residency training in Internal Medicine at Wake Forest University Baptist Medical Center, which she continues to be affiliated with as a faculty instructor in the department of General Internal Medicine and Palliative Care Service. She completed a fellowship in Hospice and Palliative Medicine in 2010 and is now a medical director at Kate B. Reynolds Hospice Home. She is board certified in Internal Medicine and Hospice and Palliative Medicine. She enjoys sharing palliative medicine with residents and medical students in our large medical centers including Wake Forest Health and Forsyth Medical Center, and communicating with patients and families about end of life issues.
Jerry Boltz, MSN, FNP, CNS

Jerry received his Family Nurse Practitioner Certificate through the Stanford Primary Care Associate Program in Palo Alto, CA. He obtained his MSN from the University of San Diego as a Clinical Nurse Specialist in Adult Medicine. His first experience in hospice was as an RN Case Manager with San Diego Hospice. After completing his Nurse Practitioner studies, he returned to San Diego Hospice, and worked as a Provider in their in-patient facility. He also worked for Nathan Adelson Hospice in Las Vegas, NV before coming to Hospice & Palliative CareCenter. Jerry is an advocate for patient determined end-of-life care.
