Position Duties and Responsibilities:  Coordinate and implement patient/family volunteer assignments.  Maintain communication with IDT regarding volunteer needs of patients and families.  Match patient/family requests with appropriate volunteers.  Attend IDT meetings as appropriate and necessary to fulfill volunteer assignments.  Provide ongoing monitoring and supervision of patient/family volunteers.  Monitor progress and success of patient/family assignments through regular communication with volunteers, with the IDT members, and with patients and families.  Conduct annual performance evaluations and competencies for all patient/family volunteers.  Assist other Volunteer Services Coordinators in the implementation of departmental events and activities, as needed.  Assist with volunteer recruitment, orientation/training and recognition.  Assist with Volunteer Services events, such as Volunteer Training classes, Brown Bag luncheons, and the annual Volunteer Appreciation luncheon.  Provide coverage for other Volunteer Coordinators, as needed.  Maintain appropriate records and statistics for volunteers in accordance with agency, state, federal and accreditation standards.  Review and maintain volunteer information files; update on a regular basis.  Review and maintain activity logs for volunteers.  Prepare volunteer activity reports, as requested.  Perform other duties as assigned by supervisor.

Position Duties and Responsibilities:  (Admission Visit) - Determine eligibility for Hospice, Home Health or Palliative Care.  Assess and evaluate the needs of the patient and family.  Educate the family about Hospice, Home Health and Palliative Care services.  Establish appropriate nursing care plans based on the needs of the patient and family, focusing on the palliation of pain and symptoms associated with the terminal illness.  Evaluate the need for other disciplines and notify appropriately.  Initiate and coordinate all resources needed to provide appropriate patient care.  Communicate with attending physician and initiate plan of care.  Assign patients to appropriate discipline.  Complete the required documentation; thoroughly and timely.  Provide direct patient care as appropriate to the visit.  (Routine visits) - Provide nursing services (treatment of symptoms, preventive measures, etc.) according to the plan of care that has been developed and in accordance with the Nurse Practice Act and Hospice Standards of Care.  Instruct the patient and family on homecare techniques.  Inform the patient’s physician of any pertinent change in their condition.  Assist in providing on-call service for the admissions department.  Provide services to the family at the time of death when needed.  Perform other duties as assigned by supervisor or the CEO.
