Position Requirements: Bachelor’s degree in Social Work from an accredited school required. Master’s degree in Social Work preferred. Minimum one (1) year’s recent experience in medial Social Work. Counseling skills with both individuals and groups. Knowledge of the community’s social, financial, and health resources. Demonstrates excellent communication skills, both oral and written. Ability to work well within an interdisciplinary team setting as well as independently with a minimum of supervision. Commitment to and understanding of the Hospice philosophy. Excellent organizational skills with the ability to handle a variety of tasks and to assign priorities to those tasks. Ability to work a flexible schedule as needed.
Position Duties and Responsibilities: Assess patient/family emotional, social and financial needs and problems. Establish goals in the patient care plan to meet the psychosocial and emotional needs of the patient and family. Provide individualized Hospice services to patients and families. Maintain ongoing support and communication with patients and families as needed, at home, at the hospital, at KBR or in long term care facilities. Provide skilled social work services to Home Health patients as needed. Provide consultation, education, and support for other members of the Hospice team, as needed. Provide assistance to the patient and family in the utilization of the appropriate community resources and programs of HPCC. Assist with Social Work on-call duties. Communicate and collaborate with the IDT regarding patient care issues. Attend IDT meetings as prepared to discuss the psycho-social and emotional needs of the patient and family and to collaborate with other team members. Assist with orientation of new HPCC staff, interns and volunteers. Explain fee for services and payment mechanisms, including the Hospice Medicare/Medicaid Benefits, for those eligible, to all new patients and families. Complete Financial Assessment when appropriate. Complete appropriate paperwork for discharges and revocations in a timely manner. Proceed with discharge planning, from KBR, in a timely manner. Complete all Admission and PR visit paperwork completely and timely. Complete clinical documentation according to agency, state, federal and accreditation standards and discipline specific standards. Complete appropriate paperwork related to discharge planning, ie FL-2, PASARR, Medicare notices. Perform other duties as assigned by supervisor.
