Position Requirements:  Current North Carolina license as a Registered Nurse and current CPR certification.  A minimum of 1 year clinical experience required, 2-3 years clinical experience preferred.  Excellent nursing assessment and problem solving skills.  Ability to work well within an interdisciplinary team setting as well as independently.  Demonstrates excellent interpersonal communication skills, both oral and written.  Excellent organizational skills with the ability to handle a variety of tasks and to assign priorities to those tasks.  Ability to work a flexible schedule.

Position Duties and Responsibilities:  Collaborate with the Hospice team to develop a plan of care with goals defined by the patient/family.  Serve as a member of the interdisciplinary team.  Attend agency wide staff meetings and discipline specific meetings as scheduled.  Adherence to state, federal, accreditation regulations, and agency policy and procedures.  Complete clinical documentation according to agency, state, federal and accreditation standards.  Supervise CNA services provided to assigned patients.  Respond to and complete clarification of data entry accurately and in a timely manner in accordance with Agency protocols.  Communicate with the interdisciplinary team and coordinate provision of patient care and resources.  Make regular home visits as ordered by the M.D. and PRN.  Assist in provision of 24-hour on-call service seven days a week.  Provide nursing services (assessment, treatment of symptoms, preventive measures, education, etc.) according to the plan of care that has been developed.  Complete the admissions and delivery of Hospice & Palliative Care services to patients living in personal residence and long term care facilities on an as needed basis.  Inform the patient’s physician of any pertinent change in their condition.  Provide services to the family at the time of death.  Practice in accordance with North Carolina State Board of Nursing, North Carolina Nurse Practice Act, and all applicable regulatory agency standards.  Provide personal care services in the absence of HHA, or in conjunction with the HHA.  Schedule daily visits to ensure appropriate management of time and mileage.  Ensure all Time Log entries are complete and verified to improve billing efficiency.  Assess need for DME and order to ensure current and future needs are met.  Establish and foster working relationships with patients, family members, professional and paraprofessional agency staff members and personnel of other agencies.  Develop contacts to educate, plan and provide optimal patient care, as well as, educate on the Hospice philosophy.  Exhibit a desire for continued personal development and growth.

Position Duties and Responsibilities:  Promoting, through experience and training, the Hospice mission and philosophy of care.  Provide in home/facility observation and training to ensure compliance with the Nurse Practice Act, Home Care and Hospice Standards of Care, clinical competencies and other pertinent clinical topics as related to the specific discipline.  Assists all agency staff in locating, identifying, and utilizing resources in medical, nursing, spiritual, and other clinical procedures and techniques.  Develop and provide discipline specific orientation including but not limited to conducting and evaluating competencies, conducting on-going assessment, evaluating training methods, and precepting staff.  Coordinate ongoing in-service education and training to homecare and KBR staff.   Coordinate and oversees annual skills lab to ensure all clinical staff meet competency for discipline specific fields.  Help to ensure clinical records are maintained in compliance with agency, state, federal and accreditation guidelines.  Coordinate medical students, nursing students and interns as appropriate to ensure a comprehensive overview of Hospice care.  Works closely with other Clinical Staff and Management to ensure quality of care and acts a resource person to all agency staff.  Provide nursing services on an as needed basis according to the plan of care that has been established and in accordance with the Nurse Practice Act and Hospice standards of care.  Assist in development, revisions and implementation of agency policies and procedures.  Serve as a member of various agency committees and coordinates improvements related to ensuring quality patient care.  Obtain and maintain certification as a CPR instructor in Basic Life Support (BLS).  Provide Electronic Medical Record training to staff.  Assist in on-call duties for admissions only.
