Community Support Program



A Community Resource
The Community Support Program (CSP) is a free non-medical service available to anyone coping with a serious illness, regardless of life expectancy and/or treatment options. The Program focuses on the emotional, financial, and spiritual needs of the client. A CSP client can be the person who has been diagnosed with a serious illness or their caregiver who is seeking information or support. The primary goal of the CSP is to provide clients and their loved ones supportive care and compassion early in the disease process.

Telephone Contact to Anyone Facing a Serious Illness
CSP services may begin at any point in the disease process. A CSP representative will contact the client on a schedule that is convenient to the client. Typically calls are bi-weekly. The CSP representative is also available during normal business hours if a client has questions or concerns.

CSP services include:
  • Assistance with accessing resources
  • Education and information
  • Emotional support to the client and/or caregivers
  • Referrals to community programs
Call the CSP representative when:
  • A diagnosis of serious illness is made
  • Goals of treatment or pain management shift
  • Symptoms become more difficult to manage
  • Treatment options change
