Living Will and Health Care Power of Attorney

The public is invited to learn about the Living Will and Health Care Power of Attorney.  Hospice & Palliative CareCenter staff will explain these documents, and you will receive a free copy of each.  This presentation will take about an hour.  A notary will be present for those ready to complete the documents.  You will need a current driver's license or other government-issued picture ID.  There is no charge for this service; donations are welcomed.  To RSVP, or for more information, please contact The Community Partnership for End of Life Care at 336-768-6157, x1622.
