SPECIALIZED CARE
The Hospice & Palliative CareCenter's Palliative Home Health program specializes in caring for individuals with a serious illness. Unlike Hospice, Palliative Home Health patients may be seeking life-prolonging, or curative therapy for their illness, yet must have a skilled nursing need.
FOR ANYONE FACING A SERIOUS ILLNESS
Although many patients have cancer as their primary diagnosis, Palliative Home Health care is also offered to individuals with Alzheimer's, congestive heart failure, emphysema and Lou Gehrig's Disease, among others.
SUPPORT FOR THE PATIENT AND FAMILY
Palliative Home Health care is most often provided in a patient's home or assisted living facility. Under the direction of the patient's primary physician, a comprehensive Palliative Home Health team cares for both patient and family. Their primary goal is to address the medical needs of patients and to keep them as comfortable as possible. Emotional and spiritual support is also offered to the patient and family so they can better understand and cope with all aspects of the illness.

HOME HEALTH SERVICES INCLUDE:
- Intermittent visits by Skilled Nurses
- Certified Nursing Assistants
- Medical Social Workers
- Volunteers
- Spiritual Care as requested
- Physical, Occupational & Speech Therapy as needed
- Assistance in obtaining medical equipment and supplies
