Since 1979, Hospice Care has been the cornerstone of the Hospice & Palliative CareCenter. Today, that same passion for providing quality care can be seen in new and expanded programs and services. When the challenges of a serious illness confront you or someone you love, support and guidance are available every step of the way.

The following is a list of Programs & Services – At A Glance.

Palliative Care
Palliative Care is compassionate, comprehensive care that focuses on improving quality of life for anyone dealing with a serious illness, at any time, regardless of diagnosis, prognosis or treatment. Palliative Care aims to reduce or eliminate pain and other physical symptoms, and assist both the patient and family with emotional and spiritual needs. It may be as simple as providing resources at the onset of a diagnosis, and as comprehensive as traditional Hospice Care.

Home Health Care
Palliative Home Health Care is a program for patients with a serious illness in need of intermittent skilled nursing care. Palliative Home Health Care patients may be seeking aggressive treatment for their illness, yet have a skilled nursing need which requires a registered nurse. Services include caring for the medical, spiritual, social and emotional needs of the patient and family.

Hospice Care
Individuals who select Hospice Care have chosen care that focuses on improving quality of life, physical and emotional comfort, dignity and peace of mind for themselves and their family. Skilled professionals most often provide services in a patient’s home including nursing homes, assisted living homes, group homes, rest homes, and retirement communities.

Pediatric Palliative Care
The Carousel Pediatric Program offers programs specifically designed to meet the needs of infants and children living with a serious illness and their families. The pediatric staff provide specialized medical, emotional, spiritual and supportive services.

Kate B. Reynolds Hospice Home
The Kate B. Reynolds Hospice Home is a 30-bed inpatient facility providing Hospice patients state-of-the-art care in a beautiful home-like setting. Hospice patients receiving care at home may be admitted to the Kate B. Reynolds Hospice Home when the necessary medical care is not easily managed at home, or when a caregiver is in need of respite. The facility is designed to meet the needs of visiting friends and families with every comfort taken into consideration.

Grief Counseling Center
Counseling services are offered to Hospice & Palliative CareCenter patients and families, as well as to anyone in the community facing the emotional challenges of losing a loved one. Individual and group counseling are available.

Community Support Program
While some clients and their caregivers are not ready for admission to Hospice Care, they can benefit from the information and support provided by our Community Support staff. The Community Support Program is led by professional staff that can make regular home visits and provide ongoing telephone consultation.

Community Partnership for End of Life Care
This is a program of the Hospice & Palliative CareCenter, joined by dozens of other community organizations that share an interest in improving end-of-life care. The primary focus of the Community Partnership is to encourage people to have conversations regarding end-of-life care with family, friends, clergy and physicians. Assistance in documenting your wishes (through completion of living wills and healthcare power of attorney forms) is provided.

Complementary Therapies Program
Complementary therapies add an extra dimension of care to Hospice & Palliative CareCenter’s commitment to nurturing the body, mind, and spirit. Trained professionals use creative arts therapies and touch therapies to promote comfort and wholeness for both patients and their families. In addition, creative arts therapists offer bereavement groups, both at hospice and throughout area school systems.
