Hospice & Palliative CareCenter created the Community Partnership for End-of-Life
Care in 1999 to help make this vision a reality. The mission of the Partnership is to
encourage conversations about end-of-life care through
community education
and outreach to
healthcare professionals.
Talking with your family and physician about hospice and palliative care openly and honestly
brings quality end of life care to patients and families much earlier. Give your family,

Education for Healthcare Professionals
Education about quality end-of-life care has been a well-kept secret. Our clinical and community educators provide on-going events
and presentations either on our campus or at your site.
On the Hospice Campus: Join us at the Williams Education & Counseling Center for CME series, Community Forums
and workshops on a variety of end-of-life care topics.
At Medical Centers we can provide speakers for Grand Rounds and CME sessions with continuing education credits
for physicians, physician assistants, nurse practitioners and RNs. We often make presentations for hospital department
staff meetings.
For Physician Practices, Long Term Care, Assisted Living sites and area human service agencies a wide range
of educational topics is available.
