The public is cordially invited.  Come learn about the advance care planning process, Living Will and Health Care Power of Attorney. Receive help in completing your documents.

Tues, June 8 at 11:00 a.m.  - Mt. Tabor United Methodist Church Fellowship Hall, 3543 Robinhood Road, Winston-Salem

Tues, June 29 at 5:30 p.m. - Williams Education and Counseling Center, Hospice & Palliative CareCenter campus

Hospice & Palliative CareCenter staff will explain these documents, and you will receive a free copy of each. A notary will be present to assist in completing the documents.
There is no charge for this service. Donations are welcome.

 Hospice Foundation of America's 17th Annual National Teleconference
"Cancer and End-of-Life Care"

You are invited to attend the Hospice Foundation of America's Annual National Teleconference for 2010.  The teleconference will be broadcast live on Wednesday, March 24, from 1:30 - 4:30 p.m.  

Continuing Education credits are available for a wide range of professions.

You may choose from two viewing locations in the Winston-Salem area: (1) WFU Outpatient Comprehensive Cancer Center, Wake Forest University Baptist Medical Center; or (2) Derrick L. Davis Forsyth Regional Cancer Center, Forsyth Medical Center.

Dr. Hammes, an international authority on advance planning for end-of-life care, will offer presentations at three Winston-Salem locations on Wednesday, March 10.  You are cordially invited to attend.

At Hospice & Palliative CareCenter and Forsyth Medical Center, he will discuss "Why Families or Physicians Keep Treating:  Lessons Learned from 600 Ethics Consults."

At Wake Forest University Baptist Medical Center, his topic will be "Creating Advance Care Plans:  What's Worked?  What's Failed?"

 Caregiver Lunch & Learn Series

11:00 a.m. - 1:30 p.m.

Free event. To be held at Senior Services, 2895 Shorefair Drive, Winston-Salem, NC.  For anyone caring for a loved one at home or in a facility.  You may attend your choice of one of these sessions:
  * Pain and Symptom Management
  * Dementia and Mental Illness
Lunch will be served at noon.  Care for your loved one is available on site during the program, but you must reserve a space when you RSVP.  For RSVP information and other details, please click here.


Advance Care Planning Course for Facilitators
Tuesdays, April 13 and 20, 2010
8:30 a.m. - 12:30 p.m.

You are cordially invited to attend our next Advance Care Planning Course for Facilitators, to be held Tuesdays, April 13 and 20, 2010.
The sessions will run from 8:30 a.m. - 12:30 p.m. (both sessions are required to complete the training) at the Williams Education and Counseling Center, Hospice & Palliative CareCenter campus, Hospice Lane, Winston-Salem.
This training is for healthcare and human service professionals, clergy, attorneys, or anyone who assists individuals in advance care planning and completion of advance directives.
