Home Health Care

The Hospice & Palliative CareCenter's Home Health Care Program specializes in caring for patients with a serious ill­ness who have a specific Skilled Nursing Need. A Skilled Nursing Need is a level of care that must be provided by a Regis­tered Nurse, or supervised by a Registered Nurse.

Home Health patients may be seeking curative therapy for their illness. Your Home Health team will be taking care of you, while also teaching your family how to care for you at home. This will help to decrease unwanted hospital visits.

Support for you & your family

Your care will be provided in your home or assisted living facility under the direction of your primary care physician. The Home Health team's primary goal is to address your medical needs. They also provide care that meets social and spiritual needs to assist you and your family in understanding your illness.

How you & your family benefit

You will receive an individualized plan for care, based on your physician's orders. Your physical, emotional and social well being will be addressed in your individualized plan. Your Home Health team will communicate with your physician on your progress and update your individualized care plan as needed.

When you are eligible for Home Health Care

You must have a Skilled Nursing Need, such as wound care, catheter care, or education related to your diagnosis.

You can be seeking curative therapy.
