Supportive Care

Your Supportive Care Partner is an expert in providing palliative care, and studies prove that having a Supportive Care Partner not only improves quality of life — it extends it.

Hospice Care

Hospice Care men walkingOur Hospice team will come to you, wherever you call home — including nursing homes and assisted living facilities. Under the direction of a primary physician, our team develops a comprehensive plan for offering emotional support and comfort to patients and families. Hospice staff are available 24 hours a day, seven days a week — tending to patients in scheduled visits as well as coming to the home when needed for special attention.

Kate B. Reynolds Hospice Home

For patients needing more intensive care than can be provided at home, the Kate B. Reynolds Hospice Home promises compassionate care in a comforting, family-like environment. We understand there are times when the challenges of an illness are too great to be managed without skilled assistance. And we understand that caring for a loved one can be overwhelming. Our Hospice Home has 40 private patient rooms offering Hospice patients state-of-the-art care in a beautiful home-like setting.

Home Health Care

The Hospice & Palliative CareCenter's Home Health Care Program specializes in caring for patients with a serious ill­ness who have a specific Skilled Nursing Need. A Skilled Nursing Need is a level of care that must be provided by a Regis­tered Nurse, or supervised by a Registered Nurse.

Home Health patients may be seeking curative therapy for their illness. Your Home Health team will be taking care of you, while also teaching your family how to care for you at home. This will help to decrease unwanted hospital visits.

Grief Counseling

Mourning the death of someone you love takes time. Your heart and mind need to find their way — in their own time. Grieving is a deeply personal and a gradual experience. The counseling we offer is about recognizing where people are in this process, helping them identify their strengths, and assisting them in developing ways to cope.

When a loved one dies, mourning is an essential step in the healing process. Counselors work with clients to validate the experience of grief, find strength and coping skills.

Complementary Therapies

Complementary therapies add an extra dimension of care to Hospice’s commitment to nurturing the body, mind, and spirit. These non-invasive, holistic practices do not replace the medical, social, or spiritual care of the hospice team. Rather, they work with the total care of the interdisciplinary team to promote comfort and wholeness for both patients and their families.

Pediatric Palliative Care

Hospice & Palliative CareCenter provides expert care to people of all ages, including infants and children. Our Pediatric Palliative Care program is specifically designed to meet the needs of infants and children living with a serious illness and their families.
